Welcome to the moderate and deep sedation information page

Please read the two policies and the guidelines for sedation under Sedation Documents.

• View the movie below.
• When finished, click on the link to the test under Sedation Documents.
• Complete the test, save it to your computer, then email it to AKMedicalStaff@Providence.org.

Once you've completed all of these steps, you may click the attestation box on your Moderate and Deep Sedation privilege request and be allowed to submit it. If the privilege form is submitted without the test being completed, it will not be accepted.
