Queen of the Valley's CARE Network provides wraparound services to Heritage House and Valle Verde residents — secure housing is health care

Napa’s Providence Queen of the Valley Medical Center understands that health begins at home. Stable housing is key to wellness, yet many in our community struggle with housing insecurity. 

To help low-income families access secure housing, Queen of the Valley Medical Center helped fund construction of Heritage House and Valle Verde Apartments, Napa’s newest affordable housing communities completed in October 2024. 

As part of Providence’s ongoing commitment to low-income residents, the Queen’s CARE (Case Management, Advocacy, Resource & Referral and Education) Network is providing wraparound on-site services such as medical support, life skills coaching, mental health counseling and substance abuse services.

This affordable housing project cost $54.2 million to develop, including a $1 million contribution from Queen of the Valley Medical Center. The opening of Heritage House and Valle Verde Apartments adds 90 affordable housing units for Napa households earning 60 percent or less of the area’s median income. Heritage House, formerly an assisted living facility, features 66 units including 58 studios and eight one-bedroom apartments. Valle Verde has 24 newly built units, including 12 one-bedroom, six two-bedroom, and six three-bedroom units. 

Providence’s CARE Network brings together hospital caregivers and community partners to assist residents with personalized care support. Nurses, social workers, mental health therapists and community health workers will assist residents with everything from scheduling health care appointments and arranging transportation to providing continued care during housing transitions. The goal is to improve residents’ health, maintain stable housing and enhance their overall quality of life.

“Providence’s investment and work with Heritage House and Valle Verde Apartments exemplifies what can be achieved when we unite to address critical community needs,” said Aura Silva, manager of care management at Providence Queen of the Valley Medical Center’s CARE Network. “However, our commitment goes far beyond initial funding.”

By providing ongoing support, Providence’s CARE Network ensures residents not only secure stable housing but also receive the care they need to maintain their health and well-being. 

“Our caregivers and partners throughout  the community recognize the importance  of providing these ongoing services that  are tailored to each client’s needs. It helps ensure client’s not only find housing but have every opportunity to thrive and become self-sufficient,” continued Silva. “Housing is health care, and Providence’s commitment goes beyond our $1 million investment to ensure clients have access to the critical services they need.”

This holistic approach helps clients and accepted applicants at both Heritage House and Valle Verde navigate complex health challenges, build independence, and ultimately thrive in their communities.

Ongoing services deliver stable and healthy futures

This work is indicative of how the Providence CARE Network operates throughout Napa County, with caregivers stationed at shelters and visiting encampments to provide wraparound treatment and care. They also coordinate with social services and field referrals from many community partners.
