Our Team

One of the many benefits of Providence Hospice of Seattle is our interdisciplinary team approach to care. Many disciplines work together with you and your family, each bringing their specialized skills to the team. Your team members include:

Primary Care Physician

This is the physician who assumes responsibility for the medical management of your treatment throughout your stay on the hospice program.

Hospice Medical Director

This is the physician who directs your hospice medical care and works with your primary care physician and the other hospice team members.

Clinical Pharmacist

The on-staff, clinical pharmacist helps ensure that the patient's medication needs are met.

Nurses

Your team will include both registered nurses and licensed practical nurses who specialize in caring for patients with life-limiting illnesses. Nurses provide the direct skilled nursing care in your home. The registered nurse (RN) also coordinates the plan of care developed by the interdisciplinary hospice team.

Clinical Social Worker

The clinical social worker assists the patient and family in coping with the psychological, emotional, social and financial realities of a life-limiting illness. The social worker can provide individual and family counseling, information regarding community resources, eligibility requirements for additional services, guidance in preparing for a home death and help with funeral arrangements.

Hospice Chaplain

The hospice chaplain provides spiritual support to patients and families. This support includes individual care to those in need, regardless of belief or religious affiliation. The chaplain can provide or arrange for religious or spiritual services when requested or appropriate.

Home Health Aides

Certified Home Health Aides provide skilled assistance with bathing and personal care activities. They may also assist with light meals and other duties as directed by the registered nurse.

Hospice Volunteers

Our hospice volunteers are an integral part of the hospice team. They are trained and supervised to assist patients and family members in a variety of ways, including: companionship, meal preparation, errands, emotional support and respite for the caregiver.

Other Hospice Services

Other hospice services are provided directly or coordinated through the hospice program. These services may include occupational therapy, complementary therapy, and other services.

Bereavement Services

Hospice care extends to family and friends who are coping with the loss of a loved one. Bereavement services support family members, friends and others who are experiencing loss and grief.
