Congestive Heart Failure Program
If you or your loved one has been diagnosed with congestive heart failure (CHF), ongoing treatment and monitoring is crucial. Our highly-trained team provides a comprehensive assessment and along with their physician develops an individual treatment plan that addresses their unique needs. Physical and occupational therapies can be added for activity tolerance and endurance training. Patients can be referred by their outpatient physician or when being discharged from the inpatient setting.
What can I expect at every visit?
- Cardio pulmonary assessment
- Assessment of patient’s symptoms and progress
- Teaching on medication regimen, diet and activity
- Communication with patient’s physician regarding progress and any changes in condition
Home Remote Monitoring
CHF patients qualify for our Home Remote Monitoring program. This allows a registered nurse to monitor blood pressure, pulse and weight remotely. Patients are alerted and receive a telephone call as soon as intervention is needed. If indicated, a nurse makes a home visit to assess and provide interventions.
