St. Joseph Health Home Health, a division of Providence Home and Community Care, has been providing medical care in patient’s home since 1988. Serving all of Orange County, our team of expert clinicians bring personalized medical care into the home.
Eligibility for home health services is dependent on your physician's order and your insurance provider. Our intake department can help you determine eligibility; call 714-712-7110. We will assist you in finding the right care.
For patients who have insurance, the coverage will vary depending on each individual plan. For patients with Medicare or Medi-Cal, coordination of benefits is arranged through our agency. St. Joseph Health Home Health provides specialized care and services to adults who require intermittent care in their home for a variety of needs. Our services include:

Quality and safety
Patient safety and quality care are of paramount concern at Providence. We have implemented a number of strategies to achieve continuous improvement in this area. Our patient safety officer combats safety challenges by developing safer procedures and educating our staff about these improved methods. In addition, we encourage our patients to participate in our efforts to increase safe practices in their home.

Making a referral for you, a family member or other loved ones is as simple as filling out the form below. Someone will respond to your referral to discuss the care needed and offer next steps to receiving care.
