Our Approach
There are many people who need to take anticoagulation medications, most often warfarin to help prevent excess blood clotting. Warfarin is commonly prescribed for people with a variety of heart conditions, and there are other medical conditions that can warrant its use, or the use of drugs like heparin, which also needs close attention. The trouble with these medications is that getting blood levels of the medications accurate takes some work, and it’s easy to throw off levels by eating certain foods, when people get sick, or as a result of using other medications.
Our clinic is staffed by a pharmacist that will work with you one-on-one to help with your Warfarin or Coumadin dosing. This service can increase your time in INR therapeutic range, increase safety, and allow you and your physician to spend more time discussing other health concerns. Your initial visit will be a 45-minute consult reviewing your medication, how you take it and diet concerns. The pharmacist will then do a small finger prick and within a minute you will know what your INR number is. Any adjustments to your dose will be done in the same visit. Follow-up visits will be 15 minutes or less.
