Have an integrity, compliance or legal concern? Use this reporting website to report any activity that appears to violate laws, rules, regulations, standards or federal health care conditions of participation or our Code of Conduct.

If you see something, say something! The Integrity Hotline is available 24/7 by calling 888-294-8455, reporting online, or through our QR code. When a report is made, a compliance team member reviews and investigates your concern, following our policies and procedures. Reports may be anonymous, and provide a method of communication with the compliance department. Every report is taken seriously and is investigated thoroughly. The integrity hotline is included in our Code of Conduct and on posters throughout our ministries/facilities.
