Providence Hospital at Home provides patients with certain health conditions the choice to receive hospital-level care at home instead of in the hospital. Patients are recommended for the program by a doctor, such as the emergency room provider.

A Snapshot into a Providence Hospital at Home’s Patient Journey
The Providence Hospital at Home care team is available 24 hours a day, 7 days a week via phone, text, and video. In addition, care team members will come to the patient’s home to support the treatment plan. The patient will have a virtual visit with the physician at least once a day, or more if indicated.
