New Patient Information

We offer a variety of services, including consultations, diagnosis and care, infusion (chemotherapy) suites, a lab and access to clinical trials. The information on this page covers basic topics that you are likely to deal with during the course of your care and treatment.

If you have additional questions, please call 360-754-3934 during regular business hours. If you need to reach a doctor outside of regular clinic hours, call 425-317-4792 to reach the on-call physician. For emergencies when the office is closed, call 425-317-4792 or go to your nearest emergency room.

Clinic appointments

Clinic visits are by appointment only. If you need to cancel or change a scheduled appointment, we would appreciate 24 hours notice. If you have been given an appointment by one of the physicians after hours, please call our office the next day to confirm your appointment time. Urgent care is available during regular office hours.

Your first visit

For your first visit to our practice, please arrive at least 15 minutes prior to your appointment so we can register you. We will also need your previous medical records and all pathology and/or radiology information. You may bring them with you or have them sent to us by your other physicians and/or labs and diagnostic centers.

Chemotherapy appointments

Physician appointments and chemotherapy appointments are tracked separately. It is important to tell the receptionist or chemotherapy nurse if you need to see your physician on your next chemotherapy visit so the schedules can be coordinated.

Laboratory test appointments

Please arrive at the laboratory 15-20 minutes prior to your appointment time.

Family conference appointments

Families of patients can schedule a conference with the patient's physician. There is a fee based on time. The fee will be charged to the family member requesting the conference.

Emergencies

For medical emergencies outside of regular clinic hours, call 800-264-9390 to reach the on-call physician. Or, go to the nearest emergency department or call 911.

Prescription refills

All prescription refills should be obtained during normal office hours. The on-call physician will provide no narcotic medication after hours.

Insurance/fees

The clinic participates in several PPO/HMO organizations and also accepts Medicare assignments and Medicaid. If your insurance requires a referral from your primary care physician, it must be obtained prior to your appointment time. Please notify our reception staff. Our staff verifies all insurance coverage on your first visit. Pre-certification and pre-authorization are provided.

Co-payments are due at the time of service, as are co-insurance amounts for all pharmaceutical expenses. All charges will be billed to your insurance carrier. Balances are due within 30 days. If your insurance coverage changes, please notify our office immediately. Failure to notify our office of insurance changes may result in non-covered charges. In this event, any non-covered charges become the full responsibility of the patient. For questions regarding your charges or your account, please call the billing staff between 10 a.m. and 4 p.m. daily.
