Formerly North Star Healthcare, we have offered Home Health Services throughout the greater Southern California area for more than 20 years. Our clinical team provides a wide range of services including nursing, therapy, social work, and home health aides.
We are a Medicare certified and Joint Commission accredited agency, and pride ourselves on exceeding all regulatory standards to meet the needs of our patients every day.
Providence Home Health Inland Empire is comprised of a highly-skilled clinical team of professionals. We offer a patient centered approach for to establish care goals and work collaboratively with your physician to maximize outcomes – all in the comfort of our patient’s homes.
Our Home Health Aide Team provides personal care services under the supervision of a Registered Nurse, including:
- Personal hygiene
- Light housekeeping
- Medication reminders
- Check pulse and respiration rate
The Occupational Therapy Team specializes in improving independence in performing activities of daily living, including:
- Dressing
- Bathing
- Transfers
- Housekeeping

- Education on joint protection techniques and exercises
Our Physical Therapy Team is dedicated to maximizing functional ability at home and specializes In providing rehabilitative care for:
- Endurance training
- TENS
- Balance / Coordination
- Safety education
- Pre- and Post-joint replacement therapy
- Osteoporosis
- Arthritis
- Neurological diseases, including stoke, Multiple Sclerosis and Parkinson’s Disease
- Post spinal surgery

The Skilled Nursing team specializes in:

The Speech Therapy team improves our patient’s ability to communicate and swallow safely. Services include:
- Establishment of speech activities
- Evaluation of emotional and physical problems in communication
- Documentation of all therapy procedures
- Evaluation of progress
- Initiation of a treatment process
The Social Services Team offers a strategic approach to our care provided and includes:
- Assist families in seeking financial assistance and other community resources
- Assist families in dealing with social and emotional problems
- Helping families follow and uderstand medical recommendations
- Collaborate with health care professionals to meet patient care goals

At North Star Healthcare, we keep our patients out of the hospital. With hospitalization rates that rank significantly better than both the state and national levels, we pride ourselves on allowing our patients to recover in the comfort of their homes without having to make those unexpected trips to the hospital for emergent care.

Services provided by our network will be billed to your insurance. We will obtain necessary authorizations from your insurance and physicians to determine home health care coverage eligibility. We will provide you with a financial responsibility letter outlining your insurance benefits and cost specific to the services rendered by our agency. This will help you make an informed decision about your care.
For patients with Medicare, Medi-Cal or Private Insurance, we will bill your insurance directly. Patient responsibility, identified by your insurance carrier, will be billed to you on a monthly basis. For patients that do not have insurance coverage, we will bill you or your guarantor directly for the services rendered. As the patient, you are ultimately responsible for payment for the services rendered.
Patient safety and quality care are of paramount concern at Providence Home and Community Care. We have implemented a number of strategies to achieve continuous improvement in this area. Our patient safety officer combats safety challenges by developing safer procedures and educating our staff about these improved methods. In addition, we encourage our patients to participate in our efforts to increase safe practices in their home.
