Your employer purchases your health insurance

Make sure the plan you select provides access to Providence.

Providence is contracted with the following HMO plans: 

  • Blue Cross of California
  • Blue Shield of California
  • CIGNA Healthcare
  • Health Net
  • United Healthcare
  • Western Health Advantage

To ensure access to Providence through these plans, you must select a primary care physician in the following medical groups or independent physician networks:

Medical group:

  • Providence Medical Group (Sonoma, Napa and Humboldt County)
    • formerly Annadel Medical Group, Queen of the Valley Medical Associates and Humboldt Medical Specialists

Independent physician networks:

Note that our hospitals and physicians may not participate in every HMO product or provider network under the health insurance plans listed. Please consult your employer or the health insurance plan to verify that our hospitals and physicians are participating in the HMO provider network or product your employer offers.

Note that the PPO or other plan products or provider networks listed are contracted with our hospitals and medical groups. Other physicians in the community who refer their patients to our hospitals for services also participate in many PPO or other plan products or provider networks which may or may not include those listed. Please consult your employer or physician to verify which PPO or other plan products or networks your physician participates in.

Also note that PPO plans sometimes have special products or networks that may not be part of the Providence network. Please consult your employer or the health insurance plan to verify that our hospitals and physicians are participating in the PPO provider network or product your employer offers.

Non - Participating

*Providence does not participate in medical discount cards (cash pay) or consumer card programs such as Care Entrée', Care IQ, Galaxy Health Network, Integrated Health Plan, MedicalConnect1, etc. that may be offered by these or any other networks.
