Please email the completed packet/forms with required signatures back to the appropriate location sites 6 weeks prior to your rotation.
All student rotations and hands-on experiences-both clinical and non-clinical must be arranged through the Education Office(s).
A clinical education agreement between the Academic Institution and Southeast Washington Providence ministries must be obtained and active prior to student onboarding. To validate the status of the facility contract, please contact the St. Mary Student Coordinator at PSMMCStudents@providence.org or Kadlec Student Coordinator at AcademicServices@kadlec.org.
All student onboarding documents/education must be completed in full prior to the student beginning their clinical experience. Please find detailed information about onboarding requirements under the Documents & Education tab.
To initiate a student rotation request, complete the following form. This can be submitted by either the student or the academic representative. A SEWA Student representative will be in contact with you regarding your request via email.
For a group request, please fill out the “Group Request” excel sheet (located on the Required Documents tab under SEWA Group Tracker). Only one Microsoft Form submission is required per group.
All students must complete the Student Intake Packet in full
Student Health Requirements (Page 8-9) must be validated by both the student and an academic representative.
All students must view the “General SEWA Healthcare Student Orientation” module. Attestation of completion is included in the student intake packet.
For students who will attend clinicals at St. Mary Medical Center (Not required if clinicals will be completed only at Kadlec Regional Medical Center) Completion of the CIA-compliance training. Register here for your unique training link to be sent to you. Once the training and post-test are completed, include a copy of your Certification of Completion with your completed packet. (This is required annually, through 5/2027.)
Once the clinical rotation is approved, and the student packet is completed, the SEWA Student representative will submit for student account access (EPIC, Pyxis, etc.) You will receive an email once your student account has been created. NOTE: You cannot claim your student account prior to your start date. Please claim your account using account claiming instructions if needed on the first day of your rotation.
Claim your Providence student account. Once your account has been claimed, additional education may be required dependent on student role. (Refer to Required Documents tab).
