When you’re admitted to Providence St. Elias Specialty Hospital, you have a team of professionals focused on helping you get well. Our case managers play a crucial role in evaluating your needs and making sure you get the most appropriate care when you’re discharged from the hospital.

Discharge planning begins as soon as you’re admitted to the hospital. We assess and evaluate your condition and what types of help or services you may need to continue your recovery after you leave our care.

Our case managers are registered nurses and licensed clinic social workers. They talk with you and your family to better understand your needs and situation. They also work closely with all members of your care team here at the hospital. Our goal is to make sure we do our best to anticipate and eliminate any barriers to your safe discharge and to make sure all your care is coordinated.
