Specialty Therapy Management Program
- The specialty therapy management program is a free service for all patients receiving a specialty medication from one of our specialty pharmacies. Some common conditions managed in this program include rheumatoid arthritis, psoriatic arthritis, Crohn’s disease, multiple sclerosis, HIV, hepatitis C and cancer. Ask us about this program!
- With this program, pharmacists can best help you answer any questions you may have about your medicine. They can also help you understand your illness, how to take your medicines, and help guide you through what to expect with your medicines. They will let you know about any possible side effects and/or drug interactions, and will connect with your doctor when necessary.
- The success of this program is limited by your level of participation. For best results, it is important that you carefully follow the directions of your doctor and pharmacist, take your medicines as directed, and be willing to discuss the details of your condition(s) and medical history with your pharmacist, so they can understand your health needs and provide the best possible care for you.
Please let your doctor know you are a patient of Credena Health and that you have signed up for the Credena Health therapy management program. A good relationship between your doctor and pharmacist will help everyone involved in your care.
