Community Health Needs Assessments and Community Health Improvement Plans

Strengthening Communities and Transforming Health Care

Every three years, hospitals across the Providence family of organizations collaborate with like-minded organizations, including local nonprofits, social services and agencies, charitable foundations, educational institutions, and many others to conduct a community health needs assessment, or CHNA. The findings from these analyses help us better understand the greatest areas of need, with a special focus on those who are poor and vulnerable.

Using data and information collected through CHNAs, hospitals and teams across the organizations collaborate with long-standing partners to develop solutions reflected in our community health improvement plans, or CHIPs. Each CHIP contains coordinated strategies responding to immediate needs, as well as root causes affecting health and well-being. For needs present across the seven states Providence ministries and affiliates serve, such as access to care or housing instability, the Providence Community Health Investment Team uses a coordinated strategy tailored to local situations.
