At Providence Medical Group, we develop a relationship with you. Your primary care provider leads your personal care team to meet your individual health care needs. Through every stage of your life, we will provide coordinated, proactive care to keep you engaged and involved in you own health.

Making your appointment over the phone is fast and easy. You will need your current insurance carrier and plan and a list of your current prescriptions. Please also be ready to answer questions about your medical history.

Preparing For Your Visit

Please arrive 15 minutes before your scheduled appointment so you have time to check in and fill out any necessary paperwork. To save time, you can print out and complete the following forms and bring them to your first appointment:

• New Patient Registration Form
• Adult Health history Form
• Pediatric Health History Form

• Photo ID and insurance card
• Referral from your primary care physician (if required by your insurance plan)
• Co-payment (if required by your insurance plan)
• Full list of medications with dosages (including over-the-counter supplements)

Insurance and Financial Information

Providence Medical Group accepts most health plans. We bill most insurance plans directly. Be sure you understand what your insurance plan covers so you are not surprised by unexpected charges. Please bring your insurance card to all appointments.

Co-pays are due at the time of service. If you are not covered by insurance, we ask for a $75 deposit at the time of your visit. We will bill you for the balance.
