Providence Hospice of Seattle partners with several excellent providers throughout King County to ensure the needs of every patient and family are met. For example, hospital discharge planners and families may struggle to find a a skilled nursing or assisted living environment that understands the unique needs of patients and families facing challenges at the end-of-life. The Providence Hospice staff has identified high quality providers in your geographic area and we communicate regularly with these providers to ensure outstanding care and satisfaction. Our partnerships with Assisted Living Communities, Skilled Nursing Homes, and Adult Family Homes ensure the following standards have been met:

• Advanced end-of-life training by Providence Hospice Nurses
• Home-like resident rooms customized to meet hospice patient and family needs
• Private family meeting space and reflection area
• Streamlined admission process
• Higher staff to patient ratio
• 24/7 medication management and care giving staff
• 24/7 Registered Nurse on-call with 5-7 days / week Registered Nurse on staff
