At Hawks Prairie Family Medicine, our experienced family practice doctors and staff offer full-service medical staff care for all ages – from newborns to seniors, and from individuals to full families.
Our clinic is specially trained to handle women's health - before, during and after pregnancy, We also specialize in preventative care for newborns, children and adolescents. Care for all ages Our clinicians have special training in the prevention and treatment of primary care concerns for adults. These can include diabetes, hypertension, high cholesterol, heart disease and other illnesses and injuries.

Prior to your first visit, please fill out the following forms. If you would like to complete the forms in the office, please arrive 15 minutes early.

To ensure that you receive the best care possible, it is very important to keep a current relationship with your provider. Typically, this occurs by seeing your provider at least once per year, if not sooner. Please keep in mind that once you are established with our practice, you will be asked periodically to complete paperwork in order to ensure that your medical record is current.
Your co-payment is due in full at the time of service; otherwise, we will need to reschedule your appointment. There are a few exceptions, such as annual physicals and injections. If you have no insurance coverage, payment is also due at the time of service. If you feel that a payment plan is necessary, please contact the business office at 866-747-2455 to make arrangements. We accept cash, check, money orders, Visa, MasterCard, American Express and Discover. Any returned checks are subject to a $25 NSF fee.
Please do not drop-in without an appointment to the office, as we are not equipped for emergent care.
The types of medications prescribed for you are ones that our physicians feel would be of most benefit to your condition. Many insurance companies place limitations on what they will cover. We will do our best to work within those guidelines, but please understand your insurance company may not cover all prescribed medicines.
Please allow 72 hours notice when you need a refill of your medicines. We ask that you contact your pharmacy directly. Please allow 24 hours for written prescriptions and samples.
Usually, all you will need to do is leave a message for the nurse. Please leave your name, phone number, and a brief message about your need with the front office staff. The nurse may have to discuss your situation with the doctor, but will get back to you as quickly as possible. We ask that you do not come into the office simply because you have not heard back from us.
We require 24 hours notice of cancellation if you are unable to keep your appointment and a minimum of one hour’s notice prior to your appointment time for same-day appointments. After three failed appointments without proper notice, you will be dismissed from our practice. We do our best to stay on schedule, so we ask that you allow for possible traffic delays, knowing we reserve the right to reschedule your appointment if you arrive more than 10 minutes late. Unfortunately, there are no exceptions.
Many insurance companies now require referrals for you to see specialists, have certain tests, or receive other forms of treatment. It is important that authorizations be in place prior to your receiving the appropriate care. Most referrals take 72 hours to process, but there are occasions where it can take longer. Also, just because we submit a referral does not mean the approval will be granted. We always suggest you check directly with your insurance company so you understand your benefits and realize which providers participate with your plan.
