Boldt Diabetes and Nutrition Education

If you don’t feel better after coming to see us, we’re not doing our jobs. We know how difficult it is to manage diabetes and never judge. Our nationally-certified diabetes educators help with understanding disease processes, managing medications, blood-sugar monitoring, dietary change, pregnancy counseling, gestational diabetes, sick day management and more. Whatever your diabetes challenges, we are here to help.

We are the only ADA-recognized diabetes clinics in Thurston County and Lewis County. Our registered nurses and registered dietitian are certified in diabetes education by the National Certification Board for Diabetes Educators.

Certified diabetes educators

A certified diabetes educator (CDE) is a healthcare professional with extensive experience working with diabetic patients. CDEs pass a comprehensive exam given by the national certification board. Before a health professional can be certified as a diabetes educator we must complete 1,000 hours of patient education experience with diabetes and pass a formal examination.

Diabetes Program

Medical nutrition therapy is taught by a registered dietitian to meet a variety of nutritional needs, including those caused by cardiovascular disorders, diabetes, obesity, renal disease, prediabetes, oncology and many others.

Continuous glucose monitoring systems were developed to create a complete picture of blood glucose levels. They track glucose levels every five minutes, or 288 times per day. The data provides important information about glucose both day and night to help you and your provider improve control of your glucose levels for better health.

Our diabetes prevention program is a one-year program for individuals with prediabetes and for those at risk of type 2 diabetes to learn how to make healthier food choices, add physical activity to their routine, manage stress, stay motivated and solve problems that can get in the way of healthy changes.

Our program includes individual and group sessions. We offer:

  • Nutrition education and individualized meal plans
  • Information about physical activity
  • Blood glucose monitoring
  • Medication information regarding types, their actions and insulin start instructions
  • Coping and problem-solving tools
  • Behavioral modification training
  • Management and prevention of acute and chronic complications
  • Sick day guidelines

Our patients who attend classes lower their A1C an average of 1.46% more than our patients who don’t come to our classes.Living with diabetes is a lifelong journey of education and growth. That is why attending classes can help you manage your health in the long run. After your first appointment, you are welcome to return for more individual education or to attend our group classes. We offer daytime and evening classes every month. The classes are taught by a registered nurse, registered dietician and an exercise physiologist. We are here for you!

Diabetes Support Group

For anyone living with or affected by diabetes. Meets the first Friday of each month, 10:30 a.m., at Stillwaters Gathering Place. Call 360-827-7699 for more information.

Diabetes Self-Management

This program includes individual assessments and group classes about nutrition, exercise, glucose monitoring, insulin pumps and complication prevention. The classes have been approved by Medicare, Medicaid and most insurance companies. We offer day and evening classes each month. Call 360-493-7567 for class schedules and fees.

Other Available Classes

Individual and group sessions are also available for gestational diabetes.

In order to bill your insurance, you will need a referral from your primary care provider.

Before your first visit, it is helpful to fill out the Diabetes Self Management Form.

On your first visit please bring:

  • Picture ID
  • Insurance cards
  • Referral from your primary care physician (if one has not already been faxed)
  • Medication and dosage: Insulin, syringes, strips, meters…everything related to your diabetes management

If you have it available, please bring:

  • Labs
  • Medical records
  • List of home blood glucose records
  • New patient paperwork
