Joining Providence in 2019, Hospice Inland Empire is a not-for-profit hospice providing services to San Bernardino, Riverside and Los Angeles Counties. We are accredited by The Joint Commission, certified by Medicare and Medi-Cal, and licensed by the state of California.
We provide a full range of hospice care and compassionate support to our patients and their families with a strong commitment to quality patient care. Our hospice care services provide comforting and empowering support to patients and families coping with end-of-life care, extending from medical and care support to grief counseling and bereavement services for family and friends.
Our comprehensive hospice care is provided wherever our patients and their families reside – Skilled Nursing Centers, Assisted Living Facilities, Residential Care Facilities for the Elderly (RCFE) and in the patient's own home. Our services are tailored to each patient and family’s specific needs.
The Hospice multidisciplinary team provides these services to patients and their families:
- Telephone access to a Hospice nurse 24-hours a day, 7-days a week
- Physician trained in hospice care
- Nursing visits to manage pain and other symptoms
- Home Health Aide who assists with personal care
- Medical social worker who provides supportive counseling as well as guidance with legal and financial issues, care options and community resources
- Spiritual counselor who offers comfort by addressing fears and concerns
- Development of an individual plan of care
- Durable Medical Equipment and medical supplies to ensure patient comfort and safety
- Medications for pain and symptom management
- Volunteers for respite, companionship and support
- Bereavement support for family members for one year following the death of a loved one
Referrals to hospice come from physicians, hospitals, family members, skilled nursing facilities and residential care facilities. A patient is appropriate for hospice when aggressive treatment has ended, and the patient chooses comfort care. The focus of care becomes one of quality of life, including pain and symptom management.
The patient is referred by the physician who oversees his or her care and who agrees with the hospice philosophy and life-limiting prognosis. Hospice team members continue to work closely with the physician throughout the course of treatment.

Hospice provides comfort and support to patients and families facing a life-limiting illness. The goal of hospice care is quality of life and meeting the physical, emotional, psychological and spiritual needs of patients. We focus on comfort.
Hospice care is provided to anyone who has a medical condition with a prognosis of six months or less if the disease or condition runs its normal course. Hospice is for patients who have chosen to focus on comfort versus curative care.
Hospice care takes place wherever you call home. This can be in the comfort of your home or in other settings such as Skilled Nursing Centers, Assisted Living Facilities and Residential Care Facilities (Board & Care Homes).
Medicare, Medi-Cal and most commercial insurance plans pay for hospice services.
At any time during a life-threatening illness when a cure is no longer possible, or the patient decides to discontinue curative care and focus on comfort. The decision to enter hospice care is made with the patient, family and their physician, preferably as soon as the focus is no longer on curing the illness. This allows for sufficient time for the hospice team to establish a relationship and manage the patient's symptoms.
Home visits by the interdisciplinary team consisting of the nurse, social worker, chaplain, certified home health aide and volunteer. Medications related to the terminal illness are covered, as well as medical supplies and medical equipment. Grief support services are provided to families for 13 months following the death.
Hospice works closely with the patient's primary care physician to create a plan of care that meets the patient’s unique needs. Patients can continue to see their primary care physician in the office setting as needed.
We serve all religions and denominational preferences. Our chaplains are educated in all faiths and religious practices. In addition, we can meet the specific needs of the Catholic community.

Did you know that there are several myths about hospice care in the community? Even health care professionals have some misconceptions about what hospice care is and who would benefit from this specialized end-of-life care.
Fact: Hospice care takes place wherever you call home, including skilled nursing homes, residential care facilities and assisted living facilities.
Fact: Hospice was designed to meet the unique needs of terminally ill patients and their families during the last six months of life.
Fact: Hospice care is available for all individuals with a life-limiting illness. It is not limited by any specific diagnosis.
Fact: The Medicare/Medi-Cal benefit and most commercial health plans cover hospice care as long as the patient continues to meet the necessary criteria. Some hospice patients stabilize and are discharged from service. These patients are allowed to come back onto service when they meet the criteria for hospice care.
Fact: Hospice patients do not have to have a DNR in place to be admitted to hospice care.
Fact: Each patient is evaluated on a case-by-case basis and receives the treatments necessary to provide support and comfort.
Fact: The Hospice Team makes intermittent visits to the patient in their home, Skilled Nursing Facility or Extended Care Facility. In addition to scheduled and “as needed” visits, a hospice nurse is available by phone 24 hours a day/7 days a week.
Fact: Medicare and Medi-Cal pay for hospice services at 100 percent coverage. Many commercial health plans now offer a hospice benefit with little out of pocket expenses to the patient or family. The financial burdens usually associated with caring for a terminally ill patient are minimal.
Fact: Caregivers at St. Mary High Desert Hospice recognize hope as a powerful, ever-changing force that continues throughout the time of living and the process of dying. Hospice offers hope that a secure, familiar care setting can be enjoyed, hope for freedom from fears of isolation, loneliness, loss of control, physical pain, and hope that your family will be nurtured and supported through comprehensive counseling services during the end-of-life journey and after a loved one’s death.

Many times, a family will inquire about making donations as a way to say “thank you” to the clinicians and staff that have compassionately cared for their loved one. One way a family can do this is to tell their friends that, “In Lieu of flowers the family asks that donations be made to St. Mary High Desert Hospice.”
When such donations are made, the family is mailed a card with the name and address of the person who made the gift. The dollar amount of the gift is not disclosed. The person who made the donation also receives a written acknowledgement that serves as their receipt. All contributions are tax deductible to the fullest extent of the law, and truly appreciated.

There are plenty of ways for you to share your gifts with our patients, families, community and staff. There are opportunities to volunteer in patients’ homes as well as in the office providing support to the team.
Do you have a special talent in the craft department? Do you own a Certified Therapy Pet? We would love for you to join our Volunteer Team.
Volunteers are interviewed and screened. Then, each Volunteer participates in comprehensive training which helps prepare them for their role on our team.
