The Providence Community Wellness Center at the Vaughn Family and Community Center in Pacoima, is located in the North East San Fernando Valley, serving Pacoima, Arleta, San Fernando and adjoining communities. The Center is committed to providing adults and families with the tools, services and resources to meet identified social, mental and health needs. The Center provides services and programs focusing on physical, spiritual, emotional and mental well-being to all in need.

Our Community Wellness Center in Pacoima serves as a wellness hub for families, organizations and community members. Facilitated by our caring, dedicated professional team we offer personalized, culturally-sensitive and linguistic-appropriate services. The center is a safe place where community is built on trust, support and commitment, healthy lifestyles habits are taught, and where change begins.
We are here to serve you with a variety of FREE services and programs to promote and reinforce healthy lifestyles, habits and positive change through:
- Exercise & Fitness (Zumba, low impact exercise groups and walking groups)
- Health Promotion & Wellness Education (Nutrition, Mental Health)
- Diabetes Prevention Program
- Support Groups
- Referrals to local services
- Community Health Insurance Program (CHIP) Our CHIP workers meet with clients in person to provide individualized assessments of eligibility for Medi-Cal, Covered CA, and Medi-Cal, guidance through the complex application process, and follow-up to assist /advocate to qualify for coverage.
