The Center for Optimal Aging is an outpatient program offered at Providence Little Company of Mary Medical Center San Pedro. The Center offers assessments performed by an expert, multi-disciplinary team comprised of physical therapists, occupational therapists, pharmacists, dietitians and a gerontology specialist, that are designed to help patients, families and caregivers understand how to safely plan and navigate any potential obstacles that may arise during the aging process.

This outpatient center features a multidisciplinary assessment team focused on the needs of older adults. Together, they develop a personalized care plan for both patients and their caregivers to help address any particular need. Your team is comprised of:

  • Physical Therapists
  • Occupational Therapists
  • Pharmacists
  • Registered Dietitians
  • Elder Care Consultants

Using all-inclusive testing and imaging, physicians at the Center for Optimal Aging conduct thorough evaluations of each patient’s individual medical conditions. These multidisciplinary and comprehensive evaluations help your team determine your medical needs and develop a health plan for your wellbeing.

Your team of medical professionals are tasked with evaluating you on a holistic level, with exams that assess you entirely, in mind, body and spirit. These assessments include tests for:

  • Strength
  • Balance
  • Mobility
  • Range of motion
  • Performance of activities of daily living
  • Cognitive performance
  • Medication use
  • Nutrition
  • Social connectivity
  • Support system

Upon completion of your assessments, your results are referred to your physician and caregivers and may include follow-up testing, referrals to specialists and community-based resource information. The Center for Optimal Aging can also connect you and your loved ones with essential information on:
