Our team offers three pathways for referring your patient to a Providence Heart Institute specialist.

1. Epic Referral: Providence and Community Connect clinics can send a referral through Epic. View a list of the referral orders.

2. Direct Messaging: For practices set up for Direct Messaging through their electronic health record system, the Heart Clinic direct address is: PROV_HEART_CLINIC_OUTSIDE_REFERRALS1225802@OC.PROVIDENCEDIRECT.ORG

3. Faxed Referral Form: If your practice is not set up for direct messaging, fax a referral form with relevant chart notes and test results.
