Our Approach

Palliative Care is a specialty that focuses on relief from symptoms and stresses of serious illness.

The goal is to improve quality of life for the patient and/or family. Referrals can be appropriate any stage in a serious illness even while receiving curative treatment. We specialize in advance care planning and advance care directives for all ages.

Appropriate outpatient palliative care referrals:

• Need for Advanced Care Planning or Code Status discussion
• Frequent hospitalizations
• Multiple co-morbidities
• Needs discussion for potential major surgical intervention or high likelihood of tertiary care
• Advance Age
• Family discussion or support
