The Heart Failure clinic at the Providence Heart Institute has been providing specialized disease management of heart failure for many years. The clinic is staffed by a multi-disciplinary group of providers who have special training in the latest medications and treatments for optimizing heart function and improving the quality of life for those patients with this condition.
The team includes a physician, advanced practitioner and cardiac-certified nurses and medical assistants, as well as a clinical pharmacist, nutritionist and enhanced access to social services. Our goal is to optimize patients’ quality of life, decrease hospital admissions and improve long-term outcomes.
