This was my first introductory meeting with Providence and PMC Da Silva Lima. My interaction at Providence, from check-in staff to meeting Da Silva Lima, was timely, efficient & professional. I walked away with the initial feelings of confidence in Da Silva Lima and services provided at Providence, Central Point. This survey is based on the first and only meeting.

Despite the turmoil at Providence this past year, I feel I have found an excellent physician. The checkin assistant and Dr answered my questions and I felt very comfortable and was reassured if anything came up that they would fit me in for care

This was my first appointment with Dr. Claude. All went well, and I have no complaints about any process that I encountered during this visit.

I have zero negative comments for any part of the Providese people from reception to surgeon. I go no where else but Provdence. I love you sll.

My provider is efficient, friendly, professional, and always available. He is quick to reply to messages when the need arises.

He has been incredibly prompt with any and all follow up lab results I've had as well as messages to him and my care team following my appointment. I couldn't recommend Dr. Da Silva Lima enough!

This visit was for establishing new primary care provider, so some of the questions asked may not be pertinent. Da Silva Lima, PA was very efficient and professional. He ordered labs because I had not had them since 2022.

I had an extremely good visit with both my new provider and the nurse practitioner who scheduled me initially - it was a nice experience to be in that office. I was very impressed with Claudemir Da Silva Lima Pa , he was very thorough, intelligent, personal. It was my first appointment with him and I was impressed.

The question requesting 0 to 10 was answered with a 9 simply because I am relatively new with this provider. He is rapidly approaching a 10. He's doing very well in all respects. No complaints.

Grateful for the kindness and respect given. Anticipate expert care will be received when needed.

Extremely glad that a provider was given, as I did not know how to begin to find a new doctor, and I definitely wanted to stay with Providence. C. Da Silva Lima, PA-C is good, however, I would have appreciated much more explanation re: my extremely high blood pressure.....THE FIRST ALARM at age 88 !!!

Claude is always so kind, thoughtful, and genuienly concerned with my problems. He explains what we need to do and why we need to do it very well.

I just moved to Oregon from the east coast. It took a while to find a provider but it was worth the wait. Claude was wonderful. He listened to me and my concerns, sent me for a CT and that showed what was causing my issues.
