First, this is the second survey I've received and completed for the [date] appointment with Dr. Santos. This past year, my family physician on 40 years, Dr. Joanna Tan, retired. She recommended Dr. Santos to be my new family physician. The transition has been very smooth. [date] was my annual physical appointment. Lab tests indicated a 6.9 A1c, which concerns me. Dr. Santos reviewed that info and other results with me. Even though I exercise daily, am a life member of Weight Watchers, I'd like to lower the A1c, so I asked for a referral to a nutritionist. Dr. Santos provided the referral, and I received a call and email within a few days. Also, Dr. Santos' receptionist and nurse are very friendly, yet professional.

I am extremely happy with my choice of changing my GP to Dr. Santos and St. Jude.

I have had a lot of pain in my legs stemming from the neuropathy In my feet. Dr. Santos truly listened to my problem as other meds have not helped. She prescribed Pregabalin and it has helped so much. Thank you Dr. Santos so very much for truly listening to me!! Although I still have pain in my legs, with this medication the pain Is not quite as bad and I can at least walk and sleep better at night! Our daughter, [Name(s)] and her best friend [Name(s)], highly recommended Dr. Santos so we're very happy we have made her our primary doctor! Thank you again Dr. Santos!!

This was a new pt/well visit. PCP very thorough. Looked at overall health, immunizations. Received tetanus catch up immunization and referral for routine lab work. MA made lab appt right from the computer the exam room, very convenient. Next week have a Zoom office visit with PCP to go over results of the blood work. Very satisfied with this visit. Thank you.

I have had a central chest hernia for many years that was stable. I was warned that if something changed, like an intestine got pinched, that could require emergency surgery. That event happened and I was able to push it back in. Then she examined the area, and ordered an ultrasound for next week. So far so good.

This care provider talked more than she listened. I was frustrated with her when I left.The office did not call me with lab results. I had to get on my chart to look at my results. She did comment on the results ASAP on the site.
