The doctor was attentive, patient, and took the time to answer all my questions. I felt genuinely cared for and well-informed about my treatment plan. The staff was friendly, and my wait time was minimal.

Receptionist, Nurse, and Doctor were pleasant, explained everything clearly and were very professional.

I appreciate that Dr. Shiozaki always listens patiently, never makes me feel rushed. He explains things in a way that I can understand and doesn't refrain from giving me important information or speaking honestly with me because the information is difficult to hear. He is also very compassionate and gentle.

I truly appreciate Dr. Shiozaki. In all the time he's been my primary care provider, he's listened to what I have to say, ordered tests necessary to diagnose and treat my conditions for the short and long term. He has never rushed the appointment and taken the time to explain everything to me in a in-depth manner that was easy to understand. He has also communicated to me through My Chart when I've received care or medications from other providers or urgent care to follow up with those appointments. I would recomend Dr. Shiozaki to anyone in a heartbeat. Sadly, I am moving to San Diego and I can only hope to find a primary care provider in that area as good as Dr. Shiozaki.

This survey asked questions of the provider and the clerks and receptionists. While I was very pleased, as usual, with the these individuals I would also like to mention the outstanding professionalism of the nurse practitioner. She has always provided outstanding assistance and support during my visits.

Dr. Shiozaki has been my valued PCP for quite some time now. I am very satisfied with his care.

My doctor knows me and makes good recommendations. I feel that I am well cared for by his nurse practitioner and I did not have to go to a lab to have my blood drawn. When I had my eye surgery last year I felt confident that I got a very good pre=opt evaluation even when my regular doctor was not available. I think very highly of Providence Healthcare services and even if I wanted to return to my previous PCP I find it difficult to leave because of the care I am receiving. Please keep it up. I have not experienced a Providence Hospital but if I needed one I definitely would source a hospital near me. I am also very happy with the Urgent Care services Ive received at the facility right next to Providence Medical Inst. Thank you.
