Social Services and Case Management at
Providence Tarzana Medical Center

 

Being hospitalized can be a difficult and confusing time. Sorting out physical and emotional problems due to an illness; making sense out of so much information; and making choices can be overwhelming. That’s why Providence Tarzana Medical Center’s Social Services and Case Management department is available to help guide patients and their families through their health care challenges.

Our medical social workers and case managers have extensive experience and skills in assisting people within the medical system. They can meet with you to discuss your concerns and help you find solutions to your medical problems.

Discharge Planning/Preparing to Go Home

You may need continuing care and special assistance at the time of your discharge. Our staff will work with you to plan ahead for this possibility. We want to make sure you are confident about managing your health care needs.

Our services are available at no charge to patients in the hospital. If we can be of service to you and your family, please ask your doctor, nurse or any hospital staff member to make a referral.

Social Services

We offer assessment, counseling, problem solving, information services and referrals to our inpatient population and their families to make sure their health care needs are met when they leave Providence Tarzana Medical Center.

Patient Assessment and Intervention

  • We assess, identify and discuss emotional and practical issues that may affect how well you cope with your illness;

  • Assist in locating community resources, specific to your needs that will be available to you after your discharge;

  • Consider positive ways to deal with changes caused by illness or disability;

  • Make sure your concerns are heard by mediating communications between you, your family and your hospital team;

  • Helping you to understand your rights as a patient and advocating your rights on your behalf;

  • Discuss and help you to gain an understanding about end-of-life choices, decision-making and available resources:
    • Hospice care
    • Living wills and advance directives
    • Grief counseling

Case Management

Patients often need assistance planning for their discharge from the hospital.

  • We work with your doctor and other members of your health care team to identify which services you may need both while you are in the hospital and when you
    go home;

  • Arrange a skilled nursing facility, rehabilitation facility or assisted living after you leave the hospital;

  • Tap into community resources for special services such as home health and medical equipment.
