Providence Rochester Family Practice provides comprehensive health services to the community. We offer full-service family practice and provide medical services for all ages.

Management of chronic diseases such as hypertension and diabetes.
Management of acute illness and injuries.
Health maintenance and health promotion such as annual sports and work physicals, pap smears, well-child examinations and immunizations.
Minor surgical procedures.
Casting and splinting.
Laboratory services.

Appointments
We are open for appointments from 8:30 a.m. to 5 p.m., Monday through Thursday, and 8:30 a.m. to noon on Friday. Our office is located at 18313 Paulson St. SW, Rochester, WA 98579.

We are closed on weekends and major holidays. After hours or weekends, if you have a medical problem that cannot wait until regular office hours, call our office at (360) 273-8818 and you will receive instructions for contacting the on-call provider. Call 911 for immediate help.

Insurance & Payment for Services
We accept most insurances, Medicare and DSHS Medicaid. Contact your insurance carrier or our office if you have questions. Co-payments and payments for uncovered services are expected at the time of service. We will be happy to work out payment arrangements.
