Our hours are Monday and Wednesday evenings, by appointment only.
We recommend that you schedule your pre-travel consultation appointment at least six weeks in advance of your departure date as it can take many weeks, and sometimes months, to complete vaccine series.
To expedite your consultation appointment, please print, complete and bring with you the intake forms. Also, please bring your immunization records, list of current medications, and itinerary with you. In addition, please read the Privacy Practice Notice. Arrive at your appointment at least 15 minutes early to process your forms. If you cannot make your scheduled appointment time, we ask that you courteously cancel at least 24 hours in advance.
