Overview
When you are admitted to a Providence facility in Oregon, you are automatically included in the patient directory. The directory is a listing of information that may include the patient’s name, location in the facility, one-word condition (such as good or fair), and religious affiliation.
If you choose to opt-out of the directory, we will not release any patient information to anyone, including friends and family. Please tell an admissions staff member or a member of your health care team of your decision any time during your stay.
