What is Hospice Care?
Hospice is a way of caring for people with a life threatening illness. The heart of Hospice care is the team. A group of dedicated health care professionals who provide comfort, care and compassion to those individuals for whom a cure is no longer possible. Comprised of the patient’s physician, a medical director, registered nurses, clinical social workers,non-denominational chaplains, home health aides, registered dietitians and trained community volunteers, the hospice team works together with the patient and their family to make the individual’s remaining months and weeks of life the most meaningful.
Who Qualifies for Hospice Care?
A person diagnosed by a licensed physician as having and advanced disease with a limited life expectancy. Referrals to TrinityCare Hospice may come from anyone, but the patient’s consent and attending physician’s authorization are required for admission. People are accepted into the Trinity Care Hospice program without regard to race, color religion, age gender, sexual orientation, disability (mental or physical) or place of national origin. While the primary goal of health care is to cure disease, there are times when it is no longer possible. When the focus of care shifts from cure to comfort, hospice offers the expert medical care and human compassion needed by most patients and families. As pain is relieved and symptoms are kept under control, the patient is better able to participate in daily life at home with family and friends. TrinityCare Hospice is reimbursed through Medicare, Medi-Cal, commercial health insurance and managed care plans. No one is denied hospice care by Trinity Care Hospice for lack of insurance coverage or ability to pay. |
