Providence Wound & Ostomy Care Services

 
Hospital based, physician referral, nurse managed out-patient clinic providing evaluation, management and education for patients with chronic wounds or ostomies.

Clinic Hours: Mon, Tues, Wed & Fri, 1-5pm
Location: PHMH, Patients seen in Same Day Surgery

 Wound Care Services

  • Assessment of causes of non-healing, chronic wounds (to include ABI & Sensorimotor for patients w/lower extremity wounds)
  • Research based, cost-effective topical wound management
  • Conservative instrumental debridement & chemical cauterization
  • Compression therapy & hose fitting
  • Patient, family & caregiver education
  • Assistance with home medical products & supplies - Including NPWT/VAC Therapy
  • Coordination with other health providers - Nutrition Services, Physical & Occupational Therapy, HME
  • Monthly referring MD written summary

 Ostomy Care Services

  • Pre-op education & site marking for colostomy, ileostomy & urostomy procedures
  • Post-op assessment with patient & caregiver support & product management
  • Assessment of stomal function, pouching system, stomal & peristomal skin concerns and patient's self-care/adaption concerns
  • Recommendations regarding ostomy care/management, peristomal skin care and prevention/management of peristomal/stomal complications (Vs Evaluate & manage skin problems associated with ostomies)
  • Patient & family education for stoma care & for incorporation of ostomy into lifestyle/ADLs
  • Coordination & education of ostomy supply options & Insurance issues

Jan Thomson, RN, BSN, WOCNJan Thomson, RN BSN WOCN
Jan has 30 years of nursing experience with the last 12 years working more specifically with Home Health patients in the management of ostomies and wound care. Jan received her BSN from the University of Vermont in 1979 and her Wound, Ostomy, Continence Nursing (WOCN) education through the New Mexico School of ET Nursing in 2000.

She participates in on-going education & networking with both Providence & Regional WOCN professional groups.
