Health Education Reimbursement
Providence Health Plan members outside of the Portland service area have access to a health education reimbursement. To qualify the following must apply:
- Be Providence Health Plan valid and active member at the time of class registration.
- Classes eligible for reimbursement are offered through Providence-contracted facilities. To locate a facility in Oregon and SW Washington view our contracted-facility list (PDF) and contact them for class information. If you are looking for a facility outside of Oregon and SW Washington find contact information for a Providence contracted hospital by going to www.multiplan.com or www.fchn.com (Washington only). You can also call the Providence Resource Line at 1-800-562-8964 with any questions.
- Payment to be paid in full by the member then complete the Providence Health Plan Health Education Reimbursement form (PDF) and follow instructions on the form to receive a reimbursement.

Classes eligible for reimbursement
Childbirth education
You are responsible for the first $25 and can be reimbursed up to $60 of any additional costs. If the class is more than $85 the member will be responsible for the balance.
Weight management
You are responsible for the first $55 and can be reimbursed up to $125 of any additional costs. If the class is more than $180 the member will be responsible for the balance.
Smoking cessation
You are responsible for the first $15 and can be reimbursed up to $160 of any additional costs. If the class is more than $175 the member will be responsible for the balance.
Members in the Portland service area
View our complete list of discount classes available at Providence facilities. Members who live in the Portland service area will not be reimbursed for classes taken through other sources. Classes offered at a Providence facility are already discounted and not available for reimbursement.
