Small Group Qualifiers
Does your employer group qualify as a "small group" for employer-sponsored health insurance with PHP? Before you login, review the following questions to see if the group is eligible for small group insurance through PHP.
Does your group meet the definition of a small group (2-50 employees*)?
- Is the group domiciled in the PHP service area?
- Is Providence Health Plans the sole health insurance carrier for this small group?
- Does the group meet the enrollment requirements of 100% of eligible employees and 60% of eligible dependent contracts? (Waiving to other group coverage does not count against enrollment requirements.)
- Are you an Oregon business that on at least 50% of working days in the past year, employed no more than 50 eligible employees and no fewer than two eligible employees?
- Do eligible employees work 17.5 or more hours per week?
* Owners are counted as an eligible employee to determine group size.
