Providence Individual & Family Plans
Providence Health Plan offers health care coverage for Oregon residents living in our service area who are under age 65 and not eligible for Medicare. Our Oregon service area includes: the Portland Metro, Willamette Valley, Hood River and Central Oregon areas. See our service area map (PDF 540kb).
Providence Health Plan is an exclusive provider organization plan offered by the Providence Health & Services and includes: a network of hospitals, clinics, urgent care centers, physicians, health plans and other health care providers. Our goal is to help improve the health status of the communities in which we serve.
Our Individual and Family Plans are available to individuals and families, and we also offer Child-only coverage.
Individuals and Families may choose from:
- Optimum Plans – five medical plan options that provide the most comprehensive coverage. Deductibles are waived for preventive care and participating health care provider and specialist office visits.
- Value Plans – five medical plan options offering the same coverage level as the Optimum plans; the deductible is not waived for participating specialist office visits.
- HSA Plans – two HSA plan options. These high deductible plans allow members to use pay for qualified medical expenses from a Health Savings Account.
All Individual and Family Plans offer a range of medical services at various benefit levels, including first dollar coverage for preventive care and many routine health care services for illness and injury. They include prescription drug benefits, chiropractic coverage, wellness benefits and access to health resources.
