Providence Choice provides peace of mind. We offer coordinated care, access to in-plan and out-of-plan doctors, and rich benefits to public employees in Oregon.

For those who live or work in the four-county Portland-metro area, Providence Choice offers access to a Providence Medical Group clinic as your medical home. These clinics are staffed by popular, community-oriented doctors and providers who offer state-of-the-art primary care and easy access to specialists.

For full-time employees, in-plan benefits include:

• $5 copay and no deductible for most physician and provider services
• $0 copay and no deductible for preventive health and women's health care services
• $0 copay and no deductible for inpatient hospital physician visits, including surgery and anesthesia
• $10 copay and no deductible on up to $1000/year in acupuncture, naturopathy and chiropractic services

Or, visit any doctor you choose outside of our plan – you pay 30 percent coinsurance for most out-of-plan services. See your benefit summaries for more details.

In-plan benefits for part-time employees and retirees include many no-cost prevention services and $30 copays and no deductible for most other physician and provider services. These members can visit any out-of-plan doctor when they pay a 50 percent coinsurance.
