Supplier Credentialing and Access

Providence has instituted a supplier credentialing program that provides security and flexibility in managing suppliers who require access to our facilities. Providence's supplier credentialing application streamlines the background check, credentialing process and the management of key information regarding the regulatory and compliance status of suppliers. Through this credentialing application, suppliers can electronically provide the information we need to:

  • Manage access to our facilities and patient care areas based on background, immunization, training and compliance status
  • Screen business partners, suppliers and representatives against state and federal sanction lists
  • Ensure we have the most accurate contact information to communicate our unique and most current business policies

Suppliers visiting Providence facilities are required to adhere to local entry requirements and check in/out using the applicable supplier credentialing application.

  • In acute care facilities, suppliers may be asked to present a printed Vendormate* ID badge in hospital facilities, at any time
  • In non-acute and administrative facilities, suppliers may be asked to present a mobile badge upon arrival and at any time during the visit
  • When visiting an off-site or professional office building, suppliers are required to comply with the check-in procedures established at such locations.

*Unless directed by local facilities to adhere to another credentialing process, we encourage suppliers to register on Vendormate.

Important information COVID-19 vaccination & testing for contractors & vendors

In accordance with applicable State or Local Health Officer Orders.

Entrance requirements
  • All individuals must be asymptomatic and wear a mask upon entering our buildings and during the entire visit.
  • Non-Fully Vaccinated individuals should utilize a N95 as their source control mask.
Proof of full vaccination (2 weeks after final shot of COVID-19)
  • COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card1) which includes name of person vaccinated, type of vaccine provided, and date last dose administered); OR
  • A photo of a Vaccination Record Card as a separate document; OR
  • A photo of the client's Vaccination Record Card stored on a phone or electronic device; OR
  • Documentation of COVID-19 vaccination from a healthcare provider; OR
  • Digital record that includes a QR code that when scanned by a SMART Health Card reader displays to the reader client name, date of birth, vaccine dates and vaccine type; OR
  • Documentation of vaccination from other contracted employers who follow these vaccination guidelines and standards.
Testing requirement for non-fully vaccinated individuals

Entrance requirements

Proof of full vaccination (2 weeks after final shot of COVID-19)

All individuals entering will need to show proof of vaccination (see acceptable documentation above) unless they meet a CA authorized exception. Final shot for the vaccination must be completed by 9/30/21.

Exceptions
  • Qualifying Medical Reason that is signed by a physician, nurse practitioner, or other licensed medical professional practicing under the license of a physician, OR
  • Based on Religious Beliefs

Testing Requirement for only those that meet an exception for vaccination or received their 2nd shot by 9/30/21 and are waiting the 14-days to be considered fully vaccinated: Follow same testing requirements above.

Proof of second vaccination (by 10/6/21)

All individuals working will have to provide proof of vaccination unless they meet a New Mexico authorized exception. Second dose for the vaccination must be completed by 10/6/21.

Exceptions
  • Qualifying Medical Condition that is signed by a physician, nurse practitioner, or other licensed medical professional licensed to practice in New Mexico, OR
  • Based on Sincerely Held Religious Beliefs
Testing requirement for those that meet an exception for vaccination
  • Provide adequate proof of a negative COVID-19 test on a weekly basis.
Which vaccines will meet Providence’s COVID-19 vaccination policy?

Any COVID-19 vaccine currently authorized for emergency use by the U.S. Food and Drug Administration will be accepted. This currently includes the two-dose Pfizer and Moderna vaccine series and one-dose Johnson & Johnson vaccine. As COVID-19 vaccines receive FDA approval, we will reassess and revise the policy, as needed, based on the science, facts and data.

Vendormate badge access update

Effective October 19, 2021 Vendors checking in/out through Vendormate that have not submitted the required COVID vaccination record or exemption will NOT receive a printed badge and access may be denied.

Requirements are subject to change to reflect the latest state and federal guidance and/or mandates. Additional communication will be issued, as needed, to help keep you and your staff up to date.

Entrance requirements effective October 18, 2021 (in accordance with applicable state/local health officer orders)

Proof of full vaccination (At least 14 days after final shot of COVID-19)

All individuals working will have to provide proof of vaccination (see acceptable documentation below) unless they meet an OR authorized exception. Final shot for the vaccination must be completed by 10/4/21.

Exceptions
  • Disability that is signed by an appropriate health care or rehabilitation professional, OR
  • Based on Sincerely Held Religious Beliefs, Practice, or Observance
Acceptable documentation

Documentation may include but is not limited to the following:

  • COVID-19 vaccination record card or a copy or digital picture of the vaccination record card, or
  • Print-out from the Oregon Health Authority’s immunization registry.
